Sheet 1: Instructions
Purpose

FMEA is a technique utilized to define, identify, and eliminate known or potential failures or errors from a product or a process
How to fill out the form

1.       Review the product, service, or process

2.       Brainstorm then sort the possible failure modes

3.       List one or more potential effect for each failure mode

4.       Assign ratings for severity (S) for each effect

5.       Determine potential causes of each failure mode

6.       Assign ratings for occurrence (O) for each cause

7.       List current monitors and controls for each failure mode

8.       Assign ratings for detection (D). This is the likelihood that the current control will detect the failure mode

9.       Calculate the Risk Priority Number (RPN). RPN = S * O * D

10.   Develop a plan to eliminate or reduce the risk for the highest risks (Mistake Proofing should come in here)

11.   Implement the plan

12.   Recalculate the RPN

Hazardous – Without Warning Endangers machine or operator. Failure will occur without warning. 10 5
Hazardous – With Warning Endangers machine or operator. Failure will occur with warning. 9
Very High Major disruption to process. 100% may have to be scrapped. Inoperable, loss of primary function. Customer very dissatisfied. 8 4
High Minor disruption to process. Product may have to be sorted (good or scrap). Operable but at reduced performance level. Dissatisfied customer. 7
Moderate Minor disruption to process. Portion may have to be scrapped. Operable but without all of the conveniences. Customer experiences discomfort. 6 3
Low Minor disruption to process. 100% may require rework. Operable, but some conveniences at a reduced level. Some customer dissatisfaction. 5
Very Low Minor disruption to process. Output may have to be sorted and a portion reworked. Fit and finish does not conform. Noticeable by most customers. 4 2
Minor Minor disruption to process. A portion may have to be reworked at rework station. Fit and finish does not conform. Noticed by average customers. 3
Very Minor Minor disruption to process. A portion may have to be reworked in station. Defect noticed by discriminating customers. 2 1
